Research interests

I led a national NPIS audit of antidote stocking in acute hospitals with showed huge variability in access to antidotes. This led to joint RCEM/NPIS guidelines on antidote availability for emergency departments in the UK and the setting up of 8 supra-regional antidote holding centres in England for category C rarely used antidotes.
I led a national study of patients treated with toxic alcohol poisoning, sponsored by EusaPharma, which showed that ethanol treatment caused more adverse reactions and medication errors compared to fomepizole. Fomepizole has been recommended as the first line treatment for toxic alcohol poisoning. Engagement with Waymade Pharmaceuticals has led to a marketing authorisation for fomepizole being granted by MHRA for treatment of ethylene glycol poisoning in the UK in 2024.
I have also served on advisory boards to SERB Pharmaceuticals and advised on dosing regimens for digoxin-specific antibodies for digoxin poisoning and indications for glucarpidase for methotrexate poisoning. I have published reviews on the optimal dosing of naloxone for opioid overdose and L-carnitine for valproic acid-induced toxicity.
I have also contributed to toxico-surveillance studies.
Paracetamol overdose
My research aims to optimise acetylcysteine treatment regimens for paracetamol poisoning to reduce adverse reactions without compromising efficacy. I devised using pharmacokinetic modelling a shorter 12h acetylcysteine regimen which was compared to the usual 21h regimen in the Scottish and Newcastle Anti-Emetic Pre-Treatment in Paracetamol Poisoning (SNAP) clinical trial. I was the Principal Investigator for the SNAP clinical trial in Newcastle 2010-2012. This showed the shorter 12h regimen of acetylcysteine reduced the incidence of vomiting and anaphylactoid reactions (Lancet 2013).
I subsequently devised a clinical SNAP protocol (Scottish and Newcastle Acetylcysteine protocol) which was endorsed by NPIS and implemented in clinical practice in several hospitals in the UK. An audit of effectiveness compared to the conventional 21h acetylcysteine regimen showed that the SNAP protocol was as effective as the conventional 21h protocol and was associated with fewer adverse reactions.(EClinical Medicine 2019).
In May 2023, the Royal College of Emergency Medicine (RCEM), National Poisons Information Service (NPIS) and British Association for the study of the Liver (BASL) issued a joint position statement recommending the SNAP regimen as the standard of care for treatment of paracetamol overdose in the UK.
I am currently involved in an international collaborative group looking to perform systematic reviews to provide consensus definitions and recommendations for the management of paracetamol overdose and chair of the HiSNAP Trial Steering Committee.
